New Patient Visit - 300
For patients who have never been seen at MPH. All new patient visits are telehealth, conducted through our secure, encrypted software. To become a new patient, fill out the request form. Our staff will then be in touch to coordinate scheduling your visit.
Follow Up Visit - 240
For existing patients of the practice. You can schedule appointments, request refills, and contact your provider in our secure patient portal.
